MAIN CONCERNS:
Always had poor teeth
- Afraid of dentists
- Continual pain, due to same teeth being restored over and over
- Unable to chew or bite or eat
- Self-conscious to smile
- No longer wants pain, hence no longer wants upper teeth
- Patient wants all work to be done whilst sedated
ON EXAMINATION:
- Numerous missing, fractured and decayed teeth
- Numerous root abscesses due to failed root canal treatments
- Impacted wisdom teeth
- Missing crowns on teeth
- Spacing between teeth
- Good vertical dimension, and freeway space, however closed down
- Patient very anxious and apprehensive
- Patient not wanting orthodontic treatment to correct lower teeth position
TREATMENT PLAN
- Plans is to increase vertical height of teeth, hence allowing patient to show more teeth
- Periodontal examination and root planning to establish bone position and quality
- Upper clearance of teeth existing including roots and replacing entire dentition with titanium implants (nine) in total
- Provisional crowns will be of acrylic and fixed to implants, hence immediate load of upper implants
- Not less than 6 months later, provisional crowns and bridges will be replaces mostly with segments of ceramic fixed to crowns and bridges
