Breast Augmentation

The procedure is performed under general anaesthetic, after carefully marking pre-operatively to ascertain the exact size, shape and position of the cavity for the implant. The incision is made peri-areolar (i.e. in the lower half of the area around the nipple). This leaves a hairline scar. Generally a pre-pectoral pocket (i.e. in front of the muscle) is created, unless the patient is extremely thin. A pre-pectoral pocket allows for better projection of the breast and prevents the complication of muscle contracture over the implant. Silicone implants are generally used (unless the patient specifies a saline filled implant) as they feel softer and more natural. A small tube drain is left in overnight to prevent blood accumulating in the cavity. Post-operatively the patient massages the breast to keep the cavity as large as possible.

Breast Lift Surgery

This is either performed through a peri-areola scar (i.e. around the circumference of the areola), or in worse cases is performed using a keyhole incision - with the resultant scar around the areola and a very small and insignificant scar in the inframammary fold. A vertical scar joins these 2 scars. All sutures are absorbable and do not need to be removed. Post-operatively the patient has to wear a bra for 4 weeks day and night.

Breast Reduction

Breast Reduction is performed under general anaesthetic, after careful pre-operative marking with the patient standing. Generally a supero-medial pedicle is performed - especially in larger breasts. The breasts are taped for six weeks post operatively to reduce tension on the scars and to obtain a narrow scar. The patient is instructed on how to tape the breasts herself for the period. A bra is worn continuously for two weeks, day and night, and then during the day for the next four weeks. All sutures are absorbable.

Brow Lift- Forehead

This is performed through three 1,5cm incisions in the hair-bearing scalp, utilizing an endoscopic technique. The brow lift can be performed under local anesthesia with sedation or combined with a facelift operation under sedation or general anesthesia. The muscles responsible for the over active frown expression are avulsed and the brow is lifted to maintain a natural aesthetic appearance, avoiding a surprised look. Stitches are removed post-operatively.

Calf Augmentation

This is performed through a small incision in the popliteal fossa (i.e. behind the knee). A custom-made calf prosthesis is inserted into the medial compartment directly on top of the gastrocnemius muscle. The procedure takes 45 minutes to 1 hour to perform, and the patient returns home the following morning. There is difficulty with walking for approximately 72 hours post-operatively as the calf is extremely tight. The patient usually requires aid and the use of shoes with a small heel is of some benefit. The sutures are absorbable and the patient is usually completely mobile after 7 –10 days.

Cheek Augmentation

Malar The Cheek Augmentation procedures are either performed on their own, via an incision in the mouth, or placed via a facelift incision. The malar prosthese are customized according to the exact aesthetic appearance that is desired. If performed as a procedure on its own the patient is receives local anesthesia and returns home the same day. Cheeck Augmentation highlights the prominence of the cheekbones and can aid in attaining a youthful look for the aging face.

 

Chin Augmentation

This is performed via an incision beneath the chin - in preference to an oral incision, which has an increased incidence of infection. A pre-formed implant (depending on the exact deformity to be corrected) is placed beneath the periosteum directly onto the mandible. The area is closed with an absorbable suture. The procedure can be perfomed on an outpatient basis under local anesthesia. Recovery is rapid.

Ear Surgery

Otoplasty Ear Surgery can be performed under local anesthesia with sedation or general anesthesia - according to the needs of the patient. The poorly formed ante-helical fold is abraded with a small instrument. Sutures are placed in strategic positions to reform the natural soft curve of the anti-helical fold. Absorbable sutures are used on the skin so they do not have to be removed. The patient wears a bandage for five days post-operatively. After five days the patient is requested to wear a bandage at night only for a period of three weeks.

Eyelid Surgery

Blepharoplasty Eyelid Surgery may involve the upper or lower eyelids, or both. The procedure in perfomed under deep sedation by an anaesthesiologist and local anaesthetic. Upper eyelid excess skin and fat is removed, as well as muscle strip if necessary. The resultant scar is a fine line which falls into the natural crease. Lower eyelid surgery can be performed as a direct surgical approach, with an incision just below the lower eyelashes (in the natural crease) to excise excess bags of fat and excess skin. Transconjunctival removal of fat bags and laser resurfacing (to tighten skin and remove fine wrinkles) is also performed.

Face-lift

A wide subcutaneous face-lift is usually performed with SMAS application (tightening of the superficial muscle system). The incision starts in the temporal hairline, progresses in a natural crease line in front of the ear, then up behind the ear hugging the earlobe and back into the hairline behind the ear. The incision behind the ear is made high up to prevent the telltale signs of a face-lift. A drain is left behind the ear overnight and the patient is wrapped up with bandages for forty-eight hours to prevent swelling and bruising. Eyelid procedures are usually performed concurrently - which will increase the length and cost of the procedure.

Laser Resurfacing

Laser resurfacing of the face is used in patients with deep wrinkles, acne scarring or sun damaged skin. A combination of Erbium laser / CO2 laser is used (Derma K laser) to precisely remove the top layers of the skin, resulting in a smooth, more youthful appearance. The skin that has been resurfaced takes approximately 7 days to heal - during which time the patient needs to cleanse the skin several times a day and apply a laser gel to seal the raw skin until it has healed. The skin then remains pink for approximately six to eight weeks. Make up can be used, and post operative consulting on skin care is provided.

Laser Resurfacing Full Face, Hands, and Neck

The carbon dioxide (CO2) laser is used for patients with severe sun damage and deep wrinkle lines, whereas the Erbium laser can be used safely on the neck and the back of the hands for pigmentation and sun spots, as well as for resurfacing facial skin on younger patients whose wrinkle lines are not too pronounced.

Erbium laser resurfacing can be performed under local anaesthetic. CO2 laser resurfacing is most often performed under local anaesthetic with sedation or general anesthesia. Both procedures are performed in a day clinic and patients return home the same day. The CO2 laser treatment takes approximately 8 –10 days to heal. The patient remains pink for a period of 2-3 months post-surgery.

Erbium laser patients are healed by the 4 -5th day, and can wear make-up at this stage. Redness will fade in approximately 2 weeks. A great advantage of laser resurfacing has been the ability to remove fat from the lower lids through the conjunctiva without leaving a scar, and the tightening of resultant loose skin (if necessary) with the laser. Patients are treated pre-operatively with a cream to prevent post-operative pigmentation, as well as with an antiviral agent to prevent cold sores developing after the laser surgery. Open dressing tequniques are employed post laser. Patients are instructed in the use of moisturizing creams, general skin care and sun protection post-surgery. It is important to read the pre and post-operative instruction sheets for laser surgery for further details.

Lip Augmentation

The filling out of the upper and lower lip has disappointingly short-term results with fat injection and collagen. However, the dermo-fat graft (i.e. a graft of the dermis with an underlying cuff of fat) has been excellent. The dermis and fat is harvested from the patient, usually from an existing scarred area such as an appendix or Caesarean Section scar. The graft is prepared, shaped and placed on top of the muscle underneath the lining of the lip through a small incision inside the mouth. This procedure is performed under local anesthesia or often as part of a facial rejuvenation procedure. The lip swells for a period of 5 - 7 days, but thereafter returns to a natural and youthful appearance.

Liposuction

Liposuction is a body contouring procedure, not a weight reduction procedure. The procedure is performed under general anaesthetic with a super-wet technique - large volumes of fluid containing adrenaline (epinephrine) and lidocaine are initially infiltrated before the procedure. This is done to cut down on post operative bruising and to assist in the aspiration to the fat. Pressure garments are used post operatively to compress the area to prevent swelling and bruising and smooth out theaspirated area. Pressure garments are used continuously (day and night) for the first ten days, and up to six weeks afterwards.

Rhinoplasty (Nose Surgery)

Correction of nasal tip, dorsal lump and broadness of nose is performed under general anaesthetic. Working through the nostril, the closed technique is usually performed. The open technique is reserved for a very difficult nasal tip correction. A splint is applied (which remains for seven days) and plugs are inserted overnight. As an in-fracture is performed bruising around the eyes is experienced in varying degrees.

Tummy Tuck (Abdominoplasty)

Different techniques are used, depending on the problem. If a small area of excess fat is found in the anterior abdominal wall, with no loose skin, liposuction is performed (see liposuction). If a small excess of skin is the problem a mini tummy tuck is performed, with or without liposuction. Lastly, a full tummy tuck involves a larger scar - which is made as a high-cut (in a natural line inside the pelvic bones). The umbilicus is repositioned after the excess skin has been removed.

Non-Surgical Procedures

Botox

Botoalinum toxin (which is medically degraded) is injected to paralyse muscles that are causing wrinkles. This works best in the frown areas and smile lines around the eyes (crow’s feet). The effect lasts approximately 6 months.

Perlane

Also known as Hyaluronic acid, which is produced in a more viscous injectable form. The treatment lasts approximately a year and can be used primarily for filling if the lips, but also to alter contour deformities in the face and to fill deep wrinkle lines, such as those of the naso-labial folds and corners of the mouth.

Restylane

Also known as Hyaluronic acid, which occurs normally in the body. Restylane is safe and is used to fill out wrinkle lines; especially in the eyelid, lip and nasal labial fold areas. Treatment lasts anywhere from 3 –6 months.
